| The Community Intervention Center defines care management as an organized process for providing a full range of appropriate treatment, rehabilitation, and support services to a consumer in a planned, coordinated, efficient, and effective manner resulting in the least restrictive living environment. The recovery process requires a health promotion system that stimulates responsibility and self-help, in which individuals define their own needs and are active collaborators with a variety of people and services. |
