In January 1998, the State Department of Health and Family Services approved Kenosha County’s application for a pilot resource center as a part of Wisconsin’s plan to redesign its long-term care system. Seven other counties developed resource centers and three of these have developed Care Management Organizations. Kenosha County’s resource center is unique in that it has two distinct components, the Developmental Disabilities Resource Center (DDRC) and
Aging and Disability Resource Center
(ADRC). The resource center portion of the redesign initiative has been very successful. The State has begun to expand the number of resource centers in the state based on the models and business practices developed by the pilot counties. The
Kenosha County Division of Disability Services has contracted with KHDS to provide the services required of the DDRC.
The DDRC performs two primary functions. The first is to provide information and assistance to individuals related to the long-term care system and other services available in the community. The other is to serve as an entry point to adult long-term care services. The DDRC staff utilizes a tool called the Long Term Care Functional Screen to determine eligibility for the funding of adult services. In 2004, the DDRC took on the task of screening children for eligibility for three new waiver programs as a part of the state’s efforts to redesign the Children’s Long Term Care system. |
