The Intake and Assessment Unit acts as the gatekeeper for the
Kenosha County Division of Disability
Services. Clients are evaluated, treatment plans are developed, and services are coordinated for Kenosha County residents who are mentally ill, developmentally disabled, or alcohol or drug dependent and who lack funding for various programs. These services may include outpatient therapy, psychiatric services, community-based treatment facilities, adult family homes, work-related services, supportive home care, inpatient services, respite, care management, etc.
Intake and assessments may occur by appointment in the office, in a hospital setting, at the
KARE Center, or in a consumer's home. In addition, this unit monitors Chapter 51/55 court proceedings and assists in the planning of aftercare treatment for court-ordered services for those persons transitioning into the community from hospitals, residential placements, and/or the KARE Center. Short-term case management may also be provided or the case may be referred to the care management unit for long-term care. |
